Chronic illness has been a passion area for me since my teenage years. Applying human centered design to healthcare is en vogue, and, yet, additional attention needs to be applied to driving value for customers that doesn’t contribute to additional burden whether they be financial, physical, or mental.

CareLink is a concept built to apply some thinking around a simple concept: how can we support patients through care coordination to reduce their stress, the time spent on managing their healthcare, and ensuring better healthcare outcomes. What if we could do this while transitioning the cost burden of this coordination to Payers, rather than Providers or Patients?
